Resource Request Form

Organizations requesting funding from St. Mark's are required to complete the Resource Request Form. You may submit your request by filling out the form below and hitting submit or you may download a fillable pdf version of the form to your computer by clicking HERE. A member of the Outreach and Mission Committee will be in touch with you as soon as possible.

The St. Mark's Outreach and Mission Committee meets on the fourth Tuesday of every month. Requests to be considered must be submitted one week prior to the meeting.
